Requirements for Incoming Freshmen

  • School Form (SF-9) or Senior High School Certificate (Original copy)

  • For Alternative Learning System (ALS):

    • Certificate of Rating of Alternative Learning System       (AF-5 Form)​

  • Certificate of Good Moral Character from the principal

  • (Original copy)

  • One piece 2x2 I.D. Picture (with nametag taken within the last six months)

  • Photocopy of PSA Birth Certificate

  • Photocopy of PSA Marriage Certificate - if applicable

  • (female student only)

  • Admission test results from the Guidance office

Requirements for First-Time Transferee

  • Transfer credentials (Honorable Dismissal)

  • Scholastic records - certificate of subjects taken with corresponding grades

  • Certificate of good moral character signed by school officials

  • One piece 2x2 photo with nametag taken within the last six months

  • Photocopy of PSA birth certificate

  • Photocopy of PSA marriage certificate - if applicable (female student only)

  • Admission test result from the Guidance Office

Requirements for Cross-Enrollees

  • Secure permit to cross enroll from your College Registrar

  • PSA Birth Certificate

  • One piece 2x2 photo with nametag taken within the last six months

  • Photocopy of PSA marriage certificate - if applicable (female student only)

  • Original transcript of record

  • Honorable Dismissal - for transfer students

  • One piece 2x2 photo with nametag taken within the last six months

  • Photocopy of PSA birth certificate

  • Admission test result from the Guidance Office

Application for Incoming Freshmen/First-Time Transferee

Step 1: Guidance Office

  • Where: 1st Level, Admin Building (J.B. Angeles Building)

  • Task: Take the Entrance Examination and secure the Admission Exam Result.

Step 2: Registrar’s Office - Window 5

  • Where: 2nd Level, Admin Building (J.B. Angeles Building)

  • Task: Submission of Requirements (refer to Admission Requirements list).

Step 3: Registrar’s Office - Window 5

  • Where: 2nd Level, Admin Building (J.B. Angeles Building)

  • Task: Obtain Preliminary Registration Form.

Step 4: Dean’s Office

  • Where:

    • BSN/PT: 3rd Level Admin Building

    • BSEd/BEEd/Supplemental: Education Building

    • BSA/BSBA/BSPA: College of Business & Accountancy Building

    • BSCS: College of Information Technology Building

    • BSHM: CHARM Building

    • BSCRIM & JD: Crim & JD Building

    • Graduate Studies: GSP Building

  • Task: Advising of Subjects.

Step 5: Encoding of Subjects

  • Where: Window 1 or Window 2

  • Task: Have your chosen subjects entered into the system.

Step 6: Fees Assessment

  • Where: Window 6

  • Task: Check how much you need to pay for tuition and other fees.

Step 7: Pay the Fees

  • Where: Window 8 (Cashier)

  • Task: Make your payment.

Step 8: Submit the Yellow Copy

  • Where: Window 7

  • Task: Give the Accounting Office their copy of the receipt.

Step 9: Submit the White Copy

  • Where: Window 3

  • Task: Give the Registrar their copy of the receipt.

Step 10: Dean’s Office (Specific Locations)

  • Where:

    • Window 10 - Collegiate (For students in undergraduate programs)

    • BSN Dean’s Office - BSN/BSPT

    • GSP Dean’s Office - Graduate Studies

    • COL Dean’s Office - Juris Doctor

  • Task: Submission of Dean’s Copy (Green Copy).
